This form is used by employers of divers to notify the Ministry of Labour prior to the start of a diving operation in Ontario. This notification should be made 48 hours prior to the start of the diving operation. The completed notice must be available at the dive location for inspection. The form will take approximately 10-15 minutes to complete. This form is also available from ServiceOntario Centres in paper format. Send one copy of the completed form to the nearest Ministry of Labour diving office. Addresses are provided on the form.
